Enhanced Case Management
At our North King County 24 x 7 Enhanced Shelter at The Oaks in Shoreline we provide a full range of case management services 24 x 7. We leverage our case management practice through partnerships with other partners, referring agencies, and North County Cities like Shoreline and Bothell.
Through our Housing and Case Management Outreach program we engage directly with those experiencing homelessness who are unhoused and also work closely with municipal departments like Police and Community Services to help triage and support their engagement and interventions. Similar to a co-responder mental health model, our Homelessness co-responder case management is highly effective for local cities working to bring services to their unhoused residents.
JustHealth Recuperative Care
In 2014-15, we operated a pilot project to gain experience in post-hospitalization healthcare services to people who lack housing and are transitioning out of an acute care hospital. The program offers minimal oversight in an overnight (motel) facility and provides support with follow-up appointments, connections to support services, and housing. Today, we are building on the experience and research data from that pilot that has informed and guided our continuing to engage support from the Health sector in both behavioral and physical health services for those we serve.
Employment Outreach
Many of our clients and participants who are unhoused, or participating in our shelter programs, are also employed. We work with job training, alternative staffing agencies, and municipal programs to increase employment opportunities and increase access to workforce development through our case management services.

Lake City Taskforce on Homelessness
Today, Lake City Partners Ending Homelessness, with two locations and over 40 staff, continues to build on the work of the Taskforce by operating shelters and outreach programs in Lake City, Shoreline, and North Seattle/King County. We continue to work closely with the community-based task force and encourage our community members to participate in our collective work through, volunteering, advocacy, and organizing work.
